The 1st House, also known as the Ascendant or House of Self, is the primary significator of the physical body and overall constitution. It represents the "hardware" you were born with, indicating your general state of the body and your resilience to disease. For instance, a Sagittarius Ascendant can indicate a strong constitution and generally good health, especially if its ruler, Jupiter, is well-placed and positively aspected. Conversely, Lunar Nodes in the 1st house are typically considered challenging to health, indicating periods of turbulence related to physical vitality and a potentially weak constitution. What it means: Traditionally the house of service and work, I think of the 6th House as your body’s maintenance bay. It’s where your daily habits, your workload, your digestion, and the thousand tiny decisions you make each day either build up or break down your health. This is where stress physiology meets real life.

What we look at:

We look at the sign on the 6th House cusp to see if you thrive on structure or get dragged down by overwork. Planets here give us more clues:

  • Jupiter here? You might have a generous appetite and output, making it important to be mindful of liver load and portion sizes.

  • Mercury here? This can indicate a very busy lane, with a tendency toward speed eating or scattered rhythms that irritate the gut.

The 6th House in medical astrology, considered a cadent and malefic house, is a primary significator for an individual's physical health, illness, and daily routines. It is the second most prominent house for illness after the 12th House. This house reveals eating habits, nutritional needs, exercise patterns, and how one maintains or neglects their well-being.

As the house governing work and daily stressors, it can indicate whether an individual is prone to overworking, possesses appropriate boundaries, or is overwhelmed by responsibilities, all of which directly impact health. It’s also the house of psychological thresholds, trauma, and our capacity for profound renewal. This is where we learn to let go of what is no longer serving us, both physically and emotionally.

What we look at:

The sign and ruler of this house show your pace and style of transformation. A signature from Pluto or Mars might indicate a powerful capacity for crisis but also a deep need for precise pacing and aftercare. We also look at aspects to the Moon (somatic memory) and Saturn (chronicity) to understand the speed limits and boundaries required for safe healing, for example.

The Clinical Translation

Working with the 8th House requires deep respect. We focus on supporting all elimination pathways: the bowel, bile, lymph, and sweat, always remembering that "low and slow" is safer. We practice grief literacy, giving ourselves permission to heal in arcs, not straight lines. We also pay strict attention to budgeting our resources—money, time, and energy—because in this house, they function like vital organ systems.

The 8th House is recognised as one of the deepest "healing" houses in medical astrology. It is centrally linked to transformation, psychological health, obsessions, addictions, and trauma. This house relates to illnesses due to circumstances beyond our control and those that force individuals to confront their mortality. Surgical procedures and biopsies often show up here.

While its primary emphasis is on psychological well-being, placements in the 8th House or the condition of its ruler can indicate physiological illnesses with a psychological or traumatic origin. The concept is that "holding on to toxic situations from the past breeds toxicity and disease in the body". Modern research supports this, showing a direct correlation between traumatic childhood experiences (ACE scores) and psychiatric and physiological illness rates.

What it means: The 12th House is the realm of what is hard to see and easy to miss. It governs hospitals, retreats, our dreams, and the subconscious. It holds the key to our sleep architecture, our sensory sensitivities, and our responses to medications. It can also point to hidden drains on our system, whether they are environmental, relational, or digital.

Neptune here can heighten sensitivity and porosity, while Saturn might point to chronic constraints. We look at the ruler of the 12th House to understand if solitude heals or isolates you, and which practices can anchor your psyche to your body.

The 12th House is a cadent and malefic house, considered the second most prominent house for illness after the 6th. It is intrinsically connected to hidden patterns, the subconscious, solitude, and spiritual health. This house governs sleep quality, dreams, and sleep disorders, highlighting a person's ability to rest and revitalise through relaxation and adequate rest.

Planets located in the 12th House are often considered physically "weakened" and psychologically "repressed" due to early childhood experiences. Because it is opposite the 6th House of health, the 12th house often shows physical illnesses, particularly chronic, difficult-to-diagnose conditions, or those with a psychosomatic component. It can also indicate periods of confinement, such as hospital stays, or times of isolation due to health issues.

The ruler of the 12th house is important to consider, especially in cases of hospitalization, psychiatric illness, and sleep disturbances or disorders, as it may "weaken" any planet it conjuncts or aspects.

• Sun or Mars in the 12th can contribute to low energy and a need for increased rest.

• Mercury in the 12th can indicate a busy mind that struggles to quiet down at night, leading to racing thoughts and difficulty falling asleep.

• Mars in the 12th may manifest as restlessness, physical agitation, and nightmares, making peaceful sleep challenging.
